Workshop F Primary Care and Behavioral Health Integration Sherrie Smith, LCSW, CGP,Assistant Vice President, RWJBH Medical Group Mental Health and Addictions Service Line RWJBarnabas Health System Course Description: Healthcare integration is more than providing access to mental healthcare and substance use treatment services. Building and sustaining integrated care means all facets of an organization must reflect the values of whole health and care collaboration, and embrace change across the workforce, including administra- tive and clinical operations. It means developing an infrastruc- ture that values behavioral health and sees it as key to systemic practice transformation, which involves mapping out care models, analyzing financial costs, and identifying revenue sources. Inte - grating behavioral health with primary care services is a critical component of integration as it ensures that patients receive a continuum of preventative and restorative mental health and addiction treatment services according to their needs over time and across different levels of the health system.This workshop will provide an overview of the RWJBH Medical Group’s in- tegrated care continuum, which ranges from collaboration, to co-location, to full integration. Learning Objectives: At the end of this session, attendees will be able to: 1. Identify integrated care models and their constructs within a larger hybrid model. 2. Analyze how models fit unique practice needs and avail - able resources while incorporating proven components of integrated health. 3. Define reasons for keeping co-management of care at the center of integrated care design. Target audience: Providers, Management, Finance Audience target levels: Beginner, Intermediate,Advanced Number and type of credits: An application has been submitted for consideration of CE credit for this course.
