The Collaborative Care Model (CoCM) is an integrated, team-based healthcare approach with a strong evidence base demonstrating its effectiveness in treating patients with co-morbid mental and physical conditions. This comprehensive framework represents a paradigm shift in psychiatric care by fostering interdisciplinary integration, improving access to behavioral health services, and focusing on measurable, patient-centered outcomes.
