NJAMHAANews: How has the affiliation with Oaks Integrated Care enabled PBHG to expand its reach to a broader client base for integrated services? Mary Pat Angelini and Tara Chalakani: The affiliation with Oaks Integrated Care has created an infrastructure of support and resources. PBHG has a footprint in 11 counties, but primarily serves Ocean and Monmouth Counties. The affiliation with Oaks broadens the reach of resources and referrals that once again, have been seamless as silos are deconstructed. Further Expanding Impact in Partnership withHorizon Through the Integrated System of Care (ISC) Program, the first of its kind in New Jersey, Horizon works with behavioral health providers to manage the complex physical, behavioral and SDoH needs of Horizon members who have serious mental illness and SUD. In addition to Acenda, Oaks and PBHG, NJAMHAAmembers CarePlus NJ, Catholic Charities-Diocese of Trenton, CPC Behavioral Healthcare and Bright Harbor Healthcare (formerly Ocean Mental Health Services) are partners in this program. “The ISC expands the holistic model to the commercial space. Horizon pulled together evidence- based practices, which allows us to go beyond what a traditional behavioral healthcare provider can do,” said Mike D’Amico from Oaks Integrated Care, which is the lead ISC agency in South Jersey. “Horizon has been an incredible partner to bring the integrated care model to commercial members. They’re very in tune with outcomes for their members, and they are partnering with us to determine the best way to deliver care,” D’Amico stated. “Horizon is a champion for making the CCBHC model sustainable,” he added. “I’m glad to see providers and payers are crafting systems that embrace integrated care. People have fallen through the cracks because the systems weren’t connected. Without warm handoffs, things don’t happen,” said Bridget DeFiccio from Acenda Integrated Care. “I hope to see more of this going forward with other payers. It’s better for the clients we’re serving. It also keeps costs down.
